feat(db): add ListEntitiesOriginatedFrom query (#6881)

* feat(db): add ListEntitiesOriginatedFrom query

Add a sqlc query to list child entities that originated from a given
parent entity, with optional filtering by entity type. This avoids
in-memory filtering of all entities when a parent filter is needed,
and unblocks the remediation work in #6570 which needs to enumerate
existing children before diffing.

Regenerate db/querier.go and the mock store, and add db tests covering
the no-children, multi-children, type-filter, and wrong-parent cases.

Closes #6880

* feat(db): add index on entity_instances.originated_from

Without this index, ListEntitiesOriginatedFrom performs a sequential
scan across all projects. Also benefits ON DELETE CASCADE, which had
the same cost when finding children of a deleted parent.
