feat: add libravatar provider (#698)

* feat: add libravatar provider

Resolves avatars from Libravatar for emails and MD5/SHA-256 hashes.
Emails follow the domain's _avatars-sec._tcp SRV record when present,
falling back to seccdn.libravatar.org. Requests use gravatarproxy=n so
Libravatar never re-serves a Gravatar image.

Placed in its own tier after gravatar/github for email and after
gravatar for hashes, so it never races or overrides a Gravatar hit.

* refactor: reuse gravatar hashing and AVATAR_SIZE in libravatar

Export toHash from gravatar and use it in libravatar instead of a second
sha256/isHash implementation. Request libravatar at constants.AVATAR_SIZE
(400) instead of a hardcoded 512, matching gravatar.
