feat: add devto provider (#692)

* feat: add devto provider

Resolve DEV (dev.to) avatars through the public Forem API, falling back
to organizations. Unwraps the media2.dev.to resize wrapper to the
original upload and treats the default placeholder as a miss.

* fix: request AVATAR_SIZE image for devto

Return dev.to's media2 resize wrapper at AVATAR_SIZE instead of the
decoded original upload. The original for ben is a 1.5 MB 1250x1250 PNG;
the 400x400 wrapper is 64 KB. Placeholder detection still runs on the
decoded original URL.
