feat: add minecraft provider (#688)

Resolve a Minecraft Java player by name or UUID through the Mojang
profile lookup API, then return the mc-heads.net face render keyed on
the canonical UUID. Unknown players miss at Mojang and resolve to
undefined, so the mc-heads Steve fallback is never served.
