* signals: re-seat a leaked lane reader in its lane (#4014)

An effect reading every row of an optimistic store spun flush forever
when the source's answer moved the optimistically written row to an
earlier index. The lane seam re-enqueued the leaked effect as a mainline
re-derivation: its first held read joined the parked transaction and was
served the staging, then its read of the written value re-entered the
lane and leaked again, every round. Flag the leaked reader
REACTIVE_LANE_DIRTY so it re-derives seated in its lane, as a computed
already is.

* signals: flag only leaked effects for their lane (#4014)

A computed is already re-seated in its lane by its override, and a
written guess's own pass belongs to the frame, so the seam only needs to
seat effects, the one reader with no lane value of its own.
