Make log output configurable and log every request (#29)

* Make log output configurable and log every request

* logback: full timestamp, no ANSI colors, LOG_FORMAT=TEXT|JSON and LOG_LEVEL
* One line per handled request through micronaut's access logger
  (LOG_REQUESTS, LOG_REQUESTS_EXCLUSIONS, probes excluded by default)
* Liquibase output is routed through logback and kept at WARN
  (LOG_LEVEL_LIQUIBASE); it printed ~90 lines on every start, partly to stdout,
  which broke JSON output. The banner is off with LOG_FORMAT=JSON
* Remove the jpa/show_sql block: without Hibernate it had no effect

* Log expected failures as one line without stack trace

Rejected requests (400, 409) are logged once with method, uri and reason instead
of the whole request and a stack trace. Only CatchAllExceptionHandler keeps the
stack trace, now at ERROR.

* Log changes of the list and registry misses

Every create, update, delete, scope grant and revocation is logged at INFO,
starting with "Issuer <did>:" and naming how many credentials were affected.
The credential types and why a lookup or change found nothing (404) are logged
at DEBUG. deleteByTrustedIssuerDidAndScope returns the number of deleted
credentials for that.

- INFO only names the issuer (and the scope): "Issuer <did> created",
  "... updated", "... deleted", "... granted credentials for scope <s>",
  "... revoked credentials of scope <s>"
- DEBUG logs the complete issuer or credentials as JSON, including claims,
  instead of extracted credential types
