fix(sqlite): keep a named in-memory database in memory

`SqliteAdapter` documents `:memory:name` as a named in-memory database,
but passed the string through to the driver as-is. SQLite only knows the
bare `:memory:`, so `:memory:name` opened a regular file of that name in
the working directory. Each name now gets its own `:memory:` handle; the
cache key keeps the names apart and `closeMemoryDatabase(name)` keeps
working.
