fix(hl7v2): keep the components of VARIES fields in segment objects

toObject() read a VARIES field (OBX-5, QPD-3, MFE-4 ...) as a primitive and
kept only its first component, so assign(toObject()) lost the rest
(QPD|...|@PID.3.1^{2232} came back as @PID.3.1). A VARIES value is now text
when it is one value; with components, an object keyed by the names of the
data type it holds - an OBX-5's by OBX-2's type - or by position where that
is not known, a component with sub-components an object the same way.
assign() writes it back, reading OBX-2 before OBX-5. The generated object
types give such fields as HL7VariesValue.
