Draw OSC 8 hyperlinks from display attributes (#1415)

A link is one more thing an AttrSpec carries, layered over the enclosing
attribute like an 'inherit' color, so linked text keeps its styling.

Refs #391

- Hyperlink(uri, *, id="") and NO_HYPERLINK; only printable ASCII is
  accepted, with the URI and id length limits VTE and iTerm2 enforce.
- AttrSpec fg/bg default to 'inherit' and take hyperlink=; copy_modified(),
  layered_over(), hash, eq and repr carry it; style_only() drops it.
- The raw screen draws links after set_terminal_properties(hyperlinks=True),
  closing them at each row end; charset "U" runs stay application-owned.
- The HTML screenshot and web screens render http, https, mailto, sftp and
  file links as <a>; web links open in a new tab. curses ignores links.
- HtmlGenerator no longer raises KeyError in true-color mode.
- escape.CSI, OSC and ST replace hand-written introducers in escape.py and
  the raw screen; the written bytes are unchanged.
- The CSS cache is keyed by style and bounded.
