docs(saml): document the metadata refresh rules and login gating

docs/security-considerations.md now asks applications to gate login
initiation, and describes the background refresh, the same-origin rule
for redirects, the refresh of expired documents and when to pin the
entity of a one-off fetch; LIMITATIONS.md, the changelog and CLAUDE.md
follow.
