* graphql: group InvalidNullError attrs under "gql"

Report InvalidNullError.Attrs as one "gql" slog group rather than
dotted gql.* keys, so gqlerror.CollectAttrs merges it with the wrapping
*gqlerror.Error's group. Use CollectAttrs in the test and docs in place
of a hand-rolled unwrap loop.

Also isolate TestNameForDir from stray Go files in /tmp, and add the
modelgen test output for out_directive_nullable_models.
