* fix: --token-count honors --max-tokens (#21 B21)

* test: in-process --token-count budget cases (#21)

* test: make the command-line path runnable in-process (run_from) and cover it

* fix: --token-count crossing file uses the real clamp helper (omitted count matches the run)

* test: allow the preview estimate to be one file off at the budget edge

* test: sweep starts above the fixed header cost

* refactor: crossing-file budget without break/multiline args (coverage attribution)
