perf: Rebuild Wii partition blocks in parallel (#183)

Rebuild each 2 MiB block of partition data in a single goroutine, with
up to GOMAXPROCS blocks running ahead of the reader and returned in
order. This replaces starting a goroutine for every chunk and sector
and waiting on them twice per block, which spent most of the CPU time
in the scheduler.

Each block is rebuilt in place: data is read into its final position,
the hashes are written alongside it and each sector is encrypted in
place. Block buffers are pooled and the CBC encrypter is reused with
SetIV, cutting allocations by 70%.

Wall time on Wii images drops by about 37%, to roughly 2.4x faster
than dolphin-tool.
