Fix three risk scoring problems found in a real report (#286)

- Score the age of the latest release in the Operational dimension only. Security
  scored the same fact again.
- Ignore unlisted versions when looking for the latest stable version. NuGet
reports 1900-01-01 as the publish date of an unlisted package, which gave a
package a maximum behind-latest penalty and a huge update lag.
- Do not ask for a license URL when the package declares its own license.
Modern NuGet packages use a license expression and have no URL.
