feat(server): Deny unmatched permissions, balance L4 over clusters

- Grant a TURN permission only on a matching ALLOW rule of the
  permission filters, so a peer no rule matches, or a server with none,
  is denied; the clusters' routers no longer take part.
- Drop the trailing DENY from the permission filter of the converter.
- Reduce the router contract to Targets, the endpoints with a port,
  domains resolved, the same for every routing policy; drop Route.
- Resolve the domains of every cluster, not only ROUND_ROBIN ones.
- Balance L4 flows over the targets of every cluster of the server
  through internal/server/balancer: a Balancer interface with one
  implementation per policy, ROUND_ROBIN only for now.
- Refuse a STDIN or FIFO conn in a TURN server with an error.
- Name operator clusters <ns>/<route>/turn/udp in the docs.
