docs: fix stale and non-compiling examples

Bring every guide, reference page and crate README in line with the 0.11
API and check their Rust snippets against it:

- bump quoted versions to ic-dbms 0.11, ic-cdk 0.20 and pocket-ic 16
- import the client from ic_dbms_client::prelude
- build update requests as structs with where_clause instead of the
  removed builder
- use Query::and_where, fields and order_by_desc
- replace DateTime::now and ic_cdk::caller, which no longer exist
- glob-import the schema crate where DbmsCanister is derived
- drive IcDbmsPocketIcClient with the nonblocking PocketIc
- fix IcDbmsAgentClient::new argument order and struct variant patterns
- drop table re-registration from the post_upgrade migration example
- correct the Candid mapping of Date, DateTime and Uuid
