fix(clean): keep a .js a hand-written .d.ts declares, and honour clean.exclude

Under src and test a .js was swept even without its .ts, so a generated
plain-JS module with a hand-written .d.ts beside it was deleted before
every build, and the build then failed on the import, far from the cause
(panates/hl7v2). Such a .js is kept now, as its .d.ts already was.

clean.exclude also reaches this sweep, not only clean.include's matches,
so a file the rules misread has a way out.
