feat: skip lockfiles by default and rank files by directory (v0.11.0) (#36)

* feat: skip lockfiles by default and rank files by directory

Lockfiles stay out of the document unless --include-lockfiles or the
matching config key is set, and a type filter no longer pulls them back
in. Category 0 is root-only, changelogs are docs, and files inside a
category are grouped by directory.
