feat: Add Intel QSV and AMD AMF hardware encoding (#160)

H.265 and AV1 encode on Intel Quick Sync or AMD AMF when no NVIDIA GPU is present; H.264 stays on libx264. Hardware is detected per codec by a one-frame trial encode and cached in settings.json, which also fixes the false CUDA detection that made every run fail over from NVENC to the CPU. QSV quality is calibrated against VMAF; AV1 NVENC/AMF QP values use the 0-255 q-index scale; settings.json is written atomically and a non-UTF-8 file no longer crashes conversions.
