fix(debugger): reconnect when the hub closes the stream (#1433)

fetchEventSource treats a cleanly ended stream as final, so the debugger stayed disconnected after the hub closed a subscription (write timeout, shutdown). Throw from onclose to go through the existing onerror retry, which resends Last-Event-ID.
