fix(refresh): preserve mounted context IDs before accepting hot updates (#3944)

A context module can re-evaluate before its HMR accept callback runs. A consumer that refreshes first then reads the new context ID while its mounted provider still uses the old one, which throws ContextNotFoundError or returns the default.

Preserve mounted contexts' IDs during module evaluation in both refresh modes, before consumers can refresh. Component replacement still waits for acceptance and hydration completion.
