Rename Include.expanded to expansion.authoritative

The flag moves into the expansion record and is named for what it
guarantees: once set, the expansion is used as is and never re-derived
from the decl. This also covers the case where re-derivation failed and
the loader's expansion is kept.

Also drop the docs from stripped decls, which nothing reads, and a
redundant field copy in fragmap.
