test: cover the multi-slice error paths and mix_slice_deblock

The multi-slice change lowered coverage by 0.4%: 46 of its 391 new or changed statements were never run, most of them in tools/mix_slice_deblock.

The new tests decode hand-built CAVLC slices that start mid-picture, run past the last macroblock, start outside the picture or hold a corrupt macroblock, and two slices into one picture. In the decoder they cover a P slice arriving before the IDR picture is complete, a stream without an IDR slice, the rejection of slice groups, and the Intra_8x8 top-left filter branches that slices in raster order cannot reach. For mix_slice_deblock they cover run, the parse errors, and a pic_order_cnt_type 0 stream from hi264's encoder, in CABAC and CAVLC, which must decode to the same frame after the rewrite.
