e-stepper: program references, grey dead frames, Clear dead frames (#152)

* feat(e-stepper): program-to-diagram arrows, lookup toggle, collapsing dead frames

Adds a display options menu to the e-stepper tab: arrows from Ref badges and
EnvBlock brackets into the diagram (off by default), a switch to leave name
lookup steps out, and a switch to collapse finished frames. Extends the CSE
diagram service with onAnchors so a host can report anchor positions.

With the host's CSE diagram, the arrows from the program are the host's
"Program references" arrow filter (reported through onAnchors); the plugin's
own options menu is only for its own diagram, where the switch is renamed to
match.

Dead frames, their data and their arrows are always drawn grey. The own
diagram gets a Clear dead frames button (until the step changes), as in the
host's CSE diagram, instead of a collapse switch.
