feat(publish): a cloudflare target, deploying to Pages or Workers once per version

A package naming "cloudflare" in publish.target deploys with wrangler:
kind "pages" uploads a directory (default dist) to a Pages project and
branch, kind "workers" runs wrangler deploy with the package's own
wrangler configuration. Each deploy is labelled with the version - a
Pages commit message <name>@<version>, a Worker version tag v<version> -
and the plan reads the label back, so a version already deployed is
up-to-date and a release run twice deploys once.

publish.skip, --dry-run and --json apply as to every target. Credentials
come from CLOUDFLARE_API_TOKEN and CLOUDFLARE_ACCOUNT_ID, and the plan
is an error without them. wrangler is the package's own when installed
and npx wrangler otherwise. Pages is checked through Cloudflare's API,
since wrangler's deployment list drops the commit message. The target
runs last, beside docker.
