Fix stale facts in AGENTS.md (#297)

* Fix stale facts in AGENTS.md

- Delegate EIP-712 schema includes `safe` and `action`; chain and Safe scope are optional.
- Delegate unique index includes `safe_id`.
- `modified` has ORM `onupdate`; repositories set the parent `modified` when adding confirmations.
- `SafeFilter` carries `chain_ids_with_nonces`.
- Origin unique index uses NULLS NOT DISTINCT.
- Fix the test node id, the `/` route, the nginx port, `uv sync --frozen` and the DB test base class.
- Use the Linear branch name and the `Queue Service` label.

* Address review comments on AGENTS.md

- List safe_id in the delegate indexes
- Explain how the parent modified is updated for transactions and messages
- /docs redirects to /, only /redoc has its own page
- Migration tests do not use AsyncDbTestCase
