feat(version): a package kept out of publishing takes no version

A package with publish.skip, or one every target that ships it leaves
out (npm's private), is now 'skip' in the version plan: no new version,
no tag, no changelog. It is not rippled into when a dependency moves and
its own change releases nothing that depends on it. version.skip decides
first, both ways: true holds back a package that publishes, false
versions one that does not. A package no publish target ships to at all
is still versioned.

This reverses the rule that version never read publish.skip: on
panates/syncbridge fourteen unpublished packages were bumped, tagged and
written into changelogs on every release.
