fix(vpa): address review feedback on prediction disruption path

- Default the PredictionEnabled feature gate to false and expose it in the KWOK chart
- Emit a deduped PredictionTargetUnresolved event when a VPA target can't be resolved
- Document that a registered prediction source is required to hydrate the store
- Add hydration and feature gate coverage for provisioning and disruption
