fix: credit a terminated instance's capacity reservation slot only once

CloudProvider.Delete calls MarkTerminated whenever the instance lookup returns NotFound, and core calls Delete
more than once after an instance is gone (node termination, then the NodeClaim finalizer). Each call added a
slot, so the cached available count for the reservation went above what EC2 reports until the next sync.
Track which instances have already been credited and credit each one once.
