Add the e-stepper protocol and runner base (#124)

common-e-stepper: channel/plugin ids, EStepperStep (a stepper step plus
environment frames, heap objects, the active frame and lookups), and the
EnvBlock/Ref program node types. runner-e-stepper: BaseEStepperRunnerPlugin,
reusing the stepper's runner base on the e-stepper's own channel.
