fix(launchers): deliver piped --stdin to revdiff; correct the sandbox settings key in docs (#378)

* docs: put the sandbox exclusion under the "sandbox" key

README, install.md and the site showed excludedCommands under "permissions",
which Claude Code does not read. #189 fixed the README and 8a4cfa8 reverted it.

* fix(launchers): deliver piped input to revdiff for --stdin

every backend starts the review from a terminal or server process, so revdiff
never inherited the launcher's stdin and rejected the pane's tty. The launcher
now spools its stdin to a file and redirects that file into the review.

* fix(launchers): create the --stdin spool private and cap its size

the spool was created through a plain redirect, so a piped diff was readable
by other users in a shared /tmp, and cat read to EOF before revdiff's 64 MiB
cap could run. It is now written under umask 077 through head -c, one byte past
the cap. The herdr preserved-pane case is recorded in docs/backlog.
