Document orderHierarchicalTerms as two-level only

The method only includes top-level terms and their direct children, which
fits its one use (Programs and Divisions in the division filter).  Terms
that are deeper, or whose parent isn't in the list, are left out.  Replace
the known-issue tests that expected otherwise with tests of this behavior.
