feat: Base64 functions, rand, Markdown TOC generation, and deferred docs-gen passes (#188)

### Added

- Deferred second-pass placeholders for docs-gen: `DOCS::DEFER($target, $args)` wraps a domain call during pass 1 expansion, minting a sentinel that `DOCS::EXPAND_DEFERRED($full_text)` replaces in pass 2 by dispatching `target($full_text, ...args)` through the live pure registry. Pass 1 checks only the envelope (the target names a `MODULE::FUNC` entry, args hold plain data) and never invokes the target; option maps ride as ordinary trailing arguments, so targets without an options map work unchanged. Sentinels frame a base64url JSON envelope in control characters around one compile-time random nonce (new `deferred_sentinel!` proc macro, invoked once per namespace), so typing the human-readable shape in a file can never trigger replacement. Unknown targets fail listing the known functions. Only `docs-gen run()` registers the new `DOCS` module; the core language never sees it.
- `MARKDOWN::TOC($md, $options)` (pure, immediate, pass-agnostic): renders a table of contents for markdown text in one call with zero sentinel knowledge, usable in any single-pass script (`LET $toc: STRING = MARKDOWN::TOC($md, {min_level: 2})`) or deferred through `DOCS::DEFER("MARKDOWN::TOC", [{...}])`. Headings parse via `pulldown-cmark` (fenced code never surfaces), anchors slug GitHub-style byte-for-byte matching the `docs_conformance` anchor check (lowercase, punctuation dropped except `-`/`_`, spaces to hyphens with runs left uncollapsed, `-1` dedup), and layouts render `tree` nested bullets or a flat `inline` bar. Options (`min_level` default 2 skipping `#` titles, `max_level`, `format`, `delimiter`) validate fail-fast at the call boundary, including the inline single-layer rule and the bare `{format: "inline"}` collapse to `min_level`.
- `MARKDOWN::PARSE($md)` (pure): immediate heading inventory returning one MAP per heading (`level`, `text`, `anchor`) for composition an... (continued)
