chore(release): merge v3 into main for v3.0.0 (#209)

* ci: run check-pr for the v3 branch

The v3 Bootstrap removal lands as pull requests into a long-lived v3
branch. check-pr only triggered for main, so those pull requests got no
CI. Run it for pull requests into v3 and pushes to v3 as well.

* feat(auth)!: move sign-in to a new filament app panel (#162)

* build(deps): require northwestern-filament-theme ^4.0

Theme v4 moves to Department Templates 4.0: the dept 4.0 fonts and
wordmark, and the required footer on by default in every panel. The
app panel's sign-in and lockdown pages rely on its footer and on the
environment indicator it now renders on simple pages.

* feat(auth)!: move sign-in to a new filament app panel

Add an `app` panel at /app as the default panel for end users. It owns
sign-in for the whole application: guests of either panel are sent to
its login page. The administration panel stays at /administration.

Sign-in becomes Filament simple pages:
- /app/login lists the configured methods: one NetID button (WebSSO,
  otherwise Entra ID) and email sign-in. When single sign-on is the
  only method it redirects straight to it, except in CI.
- /app/login/email requests and verifies a login code in two steps.
- /app/access-restricted replaces the environment lockdown page.

The login code logic moves out of the controllers into the
RequestLoginCode and AuthenticateWithLoginCode actions, keeping the
timebox, decoy challenge IDs, lockout and rate limits. The per-minute
limits were route middleware, which Livewire actions bypass, so the
actions now enforce them with the same config keys.

Panel routes don't run the web middleware group, so the app panel
applies EnvironmentLockdown itself.

BREAKING CHANGE: /auth/type, /auth/login* and
/platform/access-restricted are removed, along with the login-selection,
login-code.* and platform.environment-lockdown route names. The
administration panel is no longer the default panel, so Livewire tests
