conformance: derive resolution digests from base cells and add resolution 7 (#160)

The per-resolution digest lines were SHA-256 over the whole resolution's
stream text, which could only be produced by one process walking all
cells in order. Resolution 7 is 98.8 million cells and takes about an
hour that way, so the files stopped at resolution 6.

Redefine each resolution's stream digest as the SHA-256 of the 122
base-cell hex digests concatenated in base-cell order. The per-base-cell
lines are unchanged and remain the primary record; the resolution lines
are now derived from them, so any number of processes can produce the
base-cell lines and one cheap step combines them. The count is the sum.
Every resolution line changes under the new definition; no base-cell
line does.

Generator: the digest loop is split into digestBaseCell, a
ResolutionDigest accumulator that refuses out-of-order base cells, and
three new modes. "--digests <res> <first> <last>" writes the base-cell
lines of one slice to stdout, "--resolution-row <res>" reads base-cell
lines and writes the derived resolution line, and "--manifest" rewrites
the manifest from the files on disk. The full run also uses the on-disk
manifest writer, so the manifest covers files produced by several runs.
digests.sh drives them: one process per base cell with every online CPU
busy, rows concatenated in order, that resolution's lines replaced in
both digest files, manifest rewritten. Resolution 7 takes about seven
minutes on twelve cores. generate.sh still produces resolutions 0
through 6 in one process and does not keep resolution 7 lines, so after
a full regeneration digests.sh 7 must be run again.

Runner: TestResolutionDigests computes the base cells in parallel,
GOMAXPROCS at a time, and combines them the same way; the stream code
reports a failed call as an error instead of calling t.Fatalf from a
goroutine. Resolution 7 in Go takes a few minutes on twelve cores, with
