ci(ci): post AI reviews on PRs over the GitHub diff size limit (#7040)

The AI Review "Post review" job fails on large PRs because it fetches
the PR diff from the pulls API, which returns 406 once a diff exceeds
20,000 lines or 300 files. See the failure on #7036:

The review jobs already build the diff locally with
`generate-pr-diff.ts`, which has no size cap. The `adjudicate` job now
uploads that `pr.diff` as an artifact, and `post-review` anchors inline
comments against it instead of calling the API. Inline anchors now match
the exact diff the reviewers saw; if the PR head moves during the run,
the existing 422 fallback still folds unanchorable comments into the
review body.
