chore: emit .js/.d.ts build output and smoke-test built packages (#140)

Set `fixedExtension: false` so tsdown keeps the .js / .mjs / .d.ts / .d.mts
output names instead of .cjs / .d.cts. Package entry points are updated to
match. The CLI keeps .cjs output so its `bin` path is unchanged.

Add packages/tests/src/build.spec.ts. Unit tests resolve packages to `src`,
so nothing ran the built code. The spec loads every published package from
`build/` in a fresh Node process, as CommonJS and as ESM. It checks that the
exports match the source, namespace members included, and that the CLI
binary runs.
