Update admin1 config and fixtures for reduced global p-codes (#9)

* Update admin1 config and fixtures for reduced global p-codes

The global p-codes dataset now covers 112 countries (was 156), so KAZ005
and KAZ018 no longer exist and their mappings raised KeyError in
AdminLevel.get_pcode.

- Remove KAZ mappings
- Map French CMR names to the English-named COD and "Maputo City" to MZ11
  (both were fuzzy matching to wrong p-codes)
- Don't fuzzy match Vatovavy Fitovinany (split in 2021), Bujumbura Mairie
  or Bujumbura Rural (within the new Bujumbura province)
- Refresh global p-codes test fixture from HDX and regenerate expected
  outputs

* Don't fuzzy match AZE economic regions or parts of admin1s

AZE admin1s are now 74 rayons, so OPHI's economic regions (eg. Guba
Khachmaz) fuzzy matched to single rayons. Sub-areas such as GTM
Guatemala Municipio/Resto and SLE Western Area Rural/Urban were given
the p-code of the admin1 containing them.
