feat: add json output format

`--format json` emits the findings as JSON Lines, one object per line,
with a `kind` key that tells findings and errors apart, so that the
stream on stderr only ever contains JSON objects and can be piped into
another tool.

The reason for a non-zero exit is reported as the last object of the
stream, so all the errors go through a single format-aware path in main
instead of being printed where they happen.
