feat: add client error counter tagged by error code and mode (#1230)

This adds a counter of client connection errors, [:supavisor, :client,
:errors],
tagged with the error code and the pooling mode. It is emitted from
`ClientHandler.Error.terminate_with_error/3`.

It also fixes two exits that bypassed that path. A tcp_error, or a fatal
TLS
alert on an established connection, stopped the handler without
reporting
anything. During the handshake, both were matched by the startup packet
clause and reported as ESTARTUPMESSAGE. They are now handled as a closed
socket and reported as ECLIENTSOCKETCLOSED.
