docs: keep history out of code comments and CHANGELOG in CLAUDE.md

Code and test comments, and CHANGELOG entries, describe current behaviour. What was broken, why, and measurements go in commit messages and PR descriptions. CHANGELOG entries are one or two lines naming the affected API. Stating both in the project CLAUDE.md also guides external contributors. On #632 a doc comment justified childSizes by the cost of the loop it replaced, and the CHANGELOG entry carried the measurements.
