* clickhouse: store timestamp columns as UTC

Postgres timestamp mapped to DateTime64(6) with no time zone. ETL
writes the wall-clock value as UTC microseconds, but ClickHouse reads
a zoneless column in the server or session time zone, so text output
shifted and filtering on the source value found nothing.

Map timestamp and timestamp[] to DateTime64(6, 'UTC'), like
timestamptz. Stored bytes are unchanged.

timestamp and timestamptz now share a ClickHouse type, but Postgres
converts existing values through the session time zone when a column
changes between them, so keep rejecting that change.

Existing tables fail the layout check with a type mismatch. Document
the in-place upgrade: a metadata-only MODIFY COLUMN to the UTC type,
then recreate the __current view.

show create table prints the view's stored column list, so re-running
it as create or replace view kept DateTime64(6). Rebuild the view from
system.tables.as_select instead, which picks up the new column type.
