devkit, auto-review: allow review child threads in plan mode

Agents read plan mode's "no non-readonly tools" reminder as blocking
bb child threads and dropped to provider subagents without trying a
spawn. Plan mode keeps the project read-only; the review's brief files
in its mktemp run dir and its child threads (spawn, wait, stop,
archive, delete) are not the project.

- review-code skill: state the bb-thread tier is allowed in plan mode
  and must be attempted; the sequential tier is reached only after an
  actual failed or denied call, never pre-emptively; plan-scope "fix"
  excludes the run dir and child threads.
- auto-review plan prompts: narrow "any other file" to "any other
  project file" and add the review allowance.
- devkit review plan output: same narrowing.
- Regenerate content/index.json.
