Settings window, account menu in the sidebar, custom emojis pop-up (#281)

* Settings dialog (Profile / Appearance / Notifications) + account menu in the sidebar

Replaces the separate "Edit profile" and "Notification settings" dialogs with
one Settings dialog modelled on the Claude app: a left nav of pages, one
setting per row (label + hint left, control right), hairline dividers only
between rows, and whitespace between groups.

- Profile: photo upload (saved immediately), display name with inline
  Save/Cancel for guest accounts (SSO names stay locked), read-only email.
- Appearance: Light / Dark / Match system preview cards; panel-width reset
  (desktop only). Moved out of Edit profile.
- Notifications: desktop level select; "Different settings on mobile"
  switch that reveals the mobile level; inline keyword chips; Threads &
  mentions toggles; On this device toggles; a collapsed Troubleshooting
  section (permission, test notification, recent decisions).
- Autosave everywhere, with a shared "Saving… / Saved" indicator.
  Notification saves are queued so a burst of changes lands in order.

Account menu moves from the top-bar avatar to a footer at the bottom of
the sidebar (avatar, name, chevron) that opens upward; mobile keeps the
full-screen sheet. Menu: "Settings" replaces "Edit profile", and
"Notification settings" is gone (it's a Settings page now). Admin uses a
shield icon so the gear is unambiguous.

Also: new --color-divider token for row hairlines, a 3xl dialog size, and
dropdown separators use the divider colour in dark mode (they were
invisible on the #232323 surface).
