feat(ci): bootstrap generated repos with one-shot init workflow (#63)

- `.github/workflows/init.yml` — runs exactly once, on the instantiation push
  of a repo generated from this template (`repository.is_template == false` +
  `created == true` / `before == zeros` gate), then deletes itself:
  - rewrites `projek-xyz/template` & `projek-xyz/php-lib-template` →
    `github.repository` (via `$GITHUB_REPOSITORY` env)
  - `package.json` name → `@<repo>`, version → `0.0.0`; `composer.json`
    name → `<repo>`
  - removes template-only files: `.github/README.md`, `CHANGELOG.md`,
    lock files (regenerated with latest deps), `.agents/rules`, `init.yml`
  - uncomments the `.gitattributes` export-ignore list
  - amends the initial commit to `chore: initial commit :fire:` (committer
    from org `CI_BOT_*` vars, `github-actions[bot]` fallback) and
    force-pushes with lease
- `tests.yml`: keeps the instantiation-push gate — `GITHUB_TOKEN` can
  never create/update workflow files (verified live), so init cannot strip it; in
  generated repos it's inert (only skips a hypothetical main-recreation push)
- Readmes split: `.github/README.md` = template-facing docs (higher
  render precedence), root `README.md` = placeholder shipped to
  generated projects
