docs(postgrest): correct the bulk-insert columns doc comment (#1423)

* feat(postgrest)!: drop the Typed prefix from the typed wrappers

Rename PostgrestTypedSource, PostgrestTypedQuery and PostgrestTypedMutation
to PostgrestSource, PostgrestQuery and PostgrestMutation. The wrappers still
delegate to PostgrestRequestBuilder; the rebuild lands in the next commit so
review can tell renamed from reimplemented.

BREAKING CHANGE: the three typed wrapper types are renamed. See V3_MIGRATION.md.

Refs SDK-1568

* feat(postgrest)!: rebuild the typed wrappers on PostgrestRequest

PostgrestSource, PostgrestQuery and PostgrestMutation now hold a
PostgrestRequest value and a PostgrestClient instead of a
PostgrestRequestBuilder, and send through their own pipeline.

- PostgrestQuery drops its Phase parameter; where/order/limit/range are
  order-free and return Self.
- single() returns PostgrestQuery<R, Element> and asks PostgREST for one
object. maybeSingle() returns PostgrestQuery<R, Element?>, sends the
default array request and checks the row count on the client, per the
maybe_single_row capability spec. No isMaybeSingle flag exists.
- csv(), geojson() and explain(...) return PostgrestRawQuery, which has
no stripNulls() and no way back to PostgrestQuery.
- The builder escape hatch and init(builder:) are removed (SDK-1519
  decision superseded). PostgrestFilterableRequest requires `request`.

BREAKING CHANGE: PostgrestQuery loses its Phase parameter, and the
`builder` property and `init(builder:)` are removed from the typed
wrappers. See V3_MIGRATION.md.

Fixes SDK-1568

* docs(postgrest): correct the bulk-insert columns doc comment

Without `columns`, PostgREST does not fall back to the first row's keys and
silently drop what later rows added — it requires every row to carry an
identical key set and rejects the whole request with PGRST102 (400).
Restate both doc comments (the public `insert(_:)` overload and the private
`columnsQueryItem` helper) accordi... (continued)
