fix: Prevent meta-schema collision when external schema uses https:// draft URI (#463)

AJV registered draft-06/07 meta-schemas under http:// keys but external
schemas (e.g. Adaptive Card) declare $schema with https:// URIs. On
compileAsync, AJV called loadSchema for the https:// variant which then
tried to add a schema whose $id resolved to the same http:// key,
causing "schema with key or id already exists".

Fix: intercept draft-06/07 URIs in loadSchema callbacks and return the
bundled meta-schema directly, bypassing the network fetch and the
duplicate-registration error.

Also replace the incorrect schema-1.69.0.json fixture (used in the
1.49.0 integration test) with the real draft-07 schema-1.49.0.json,
update adaptive-card.json to the real https:// variant from
adaptivecards.io, and add unit tests covering both http:// and https://
meta-schema URI variants.
