config: reduce effective config allocations

Reduce cold-start work and allocations when resolving effective configs.

- Index only immutable source subtrees needed by the requested entity.
- Allow concurrent index reads and keep variable providers outside locks.
- Replace regexp matches with compact offsets and avoid string pointer allocation.
- Preallocate child keys and clone ordered child tables directly.
- Test cache bounds, concurrent reads, clone isolation and parser equivalence.
