market_data: fix generic_tick ETF NAV constants (#946)

ETF_NAV_FROZEN_LAST was "578", which IB maps to ticks 92/93 (NAV close,
prior close), not tick 97 (frozen NAV last, generic tick 623).

Add ETF_NAV_CLOSE ("578") and ETF_FROZEN_NAV_LAST ("623"); deprecate
ETF_NAV_FROZEN_LAST as an alias of ETF_NAV_CLOSE. ETF_NAV_BID doc names
ticks 94 and 95; module docs note 576/578 come from IB's earlier table.
