Restructure on wrouesnel/golang-template

Adopt the golang-template layout and tooling:

- mage build (binary, test, lint, style, release targets), golangci-lint
  config, and GitHub workflows for CI, CodeQL, and tag-driven releases.
  The release job also publishes SHA256SUMS.
- The application lives in pkg/entrypoints/ssm_proxycommand with a
  minimal cmd/ main; version info in version/.
- zap logging via logutil, ctxstdio for stdio, and afero/pathlib for
  reading AWS profile files.

Deviations from the template, for a ProxyCommand:
- No configuration file (ssh runs it from an arbitrary directory) and
  no container image.
- Logs default to warn, and command errors print plainly to stderr,
  since ssh shows stderr in the user's terminal.
- Environment variables use SSM_PROXYCOMMAND_*: the template's
  DefaultEnvars(version.Name) produced invalid names for hyphenated names.
- SIGHUP also ends the session.
